一、您所填寫的個人資料係主辦單位(衛生福利部健康大數據治理應用計畫及國家衛生研究院)所蒐集、處理及利用，蒐集之目的僅供辦理本研討會之相關業務所需。
二、蒐集之個人資料項目：個人的姓名、機構、單位、職稱、電子郵件地址、聯絡電話、身分證字號或居留證號碼等個人訊息。
三、您所填寫的個人資料使用於辦理本研討會業務所須之執行期間，由主辦單位執行本次研討會之必要相關人員使用，所蒐集之個人資料僅用於活動當天資料(活動身分識別、簽到/退表單)及辦理活動相關庶務，使用人員需依執行本研討會作業所必要方式利用此個人資料，在未獲得您的同意前絕不會將您的個人資料使用於蒐集目的以外之其他用途。
四、本研討會採網路報名方式，若報名資料有錯誤及變動，請聯繫主辦單位；另，依個人資料保護法第3條規定得行使之權利及方式，活動參與者於身分獲確認後，得向由主辦單位提出申請查詢或請求閱覽、製給複製本、補充、更正、停止蒐集、處理、利用或請求刪除個人資料全部或部分內容，如您欲行使上述行為，請務必於主辦單位執行相關必要業務前（114年10月29日前）來電告知並更正。洽詢電話037-206-166 分機36194
